What is a bladder diary? A bladder diary is a simple chart which allows you to record the fluid you drink and the urine you pass during the day and night, as well as document if you have urgency (a sudden and intense need to pass urine that cannot be put off) and/or accidental urine leakages.
The bladder diary should be used to track your drinking, urination and leakage patterns for three to seven days in succession. If you go to work, you should have a separate copy to keep at the office. Both copies should be taken when you report to your doctor.
In the time column, please write BED when you went to bed and WOKE when you woke up. Drinks - Write the amount you had to drink and the type of drink. Urine output - Enter the amount of urine you passed in millilitres (mls) in the urine output column, day and night. Any measuring jug will do.
Measure liquid in a cup one time and then use the same cup for drinking during the three days. Write the time and amount you urinate. If you have an incontinent episode, record it in the appropriate box and try to describe the amount, e.g., drips, changed panty liner, changed underwear and pants, etc.

Before you begin buy a small measuring jug to help measure the your pee. Each time you go to the toilet record how much urine you passed and when, noting whether you had a sudden urge to pee or not. Also record the time you pass urine at night.
For urgency: write down how strongly you felt the urge to empty your bladder scoring 1 slight urge, 2 strong urge, 3 very hard to hold on. For leakage: any time you have a urine leak, whatever the amount, write down the time and how wet you felt.
