2. In Part 1, enter the registration district and entry number. If any information is not recorded, indicate 'not recorded on certificate'.
3. Fill in the place of birth and date of birth using black ink and capital letters. Ensure any corrections are initialed, avoiding correction fluid.
4. Select the gender by ticking either 'Male' or 'Female'. Enter the registered name; if a first name is not available, use the surname and gender.
5. Complete the name of the registrar and date of registration. If the signature is illegible, note that as well.
6. In Part 2, certify that you have examined the birth certificate by filling in your address and postcode. Include your full name, position, and date.
7. Sign where indicated and ensure a countersignature from a senior official if required. Finally, add your firm’s stamp.
