Please note: A request for records of services provided up to 6 years ago is a 30-day process. All other requests have a 60-day time frame for additional processing.
Online Medical Records Request Through MyUCSDChart Log in to MyUCSDChart. Go to Menu (top left) and choose Medical Records(in My Records section). Choose Medical Record Request and follow the prompts. Your records will be delivered to your MyUCSDChart account, usually within three business days.
