If you visit the Motor Vehicle Commission for a temporary placard, youll be directed to your local police station. Simply fill out the Application for Temporary Placard, which is Form SP-68. You must include medical certification and pay a $4 fee to complete your application.
If you do not receive your renewal form, you may visit any Motor Vehicle Commission Agency or call the MVC at 609-292-6500 or (tollfree in NJ) 888-486-3339. Once completed, visit your local MVC Agency. There is no fee for the identification card or placard.
