ATTACHMENT 1 Proof of Identity Checklist for Vendors or 2026

2. Begin by entering the vendor name and property address at the top of the form.
3. For Primary Proof of Identity, select one document from the list provided. Place an ‘X’ in the box next to the document you are submitting, such as a current Australian photo driver’s licence or passport.
4. Fill in the details of the document provided, including Document Type, Document Number, Date of Issue, Expiry Date, and Place of Issue.
5. For Secondary Proof of Identity, choose two documents from the specified list. Again, mark an ‘X’ next to each document you are providing.
6. Ensure that at least one document from each section is original or certified copies as required.
7. Complete the declaration section by printing your name and signing as the agent after confirming all documents have been sighted.
