2. In Section I, enter the names of the lawful custodial parent(s) or legal guardian(s) in the provided space.
3. Proceed to Section II and fill in the minor's full name, date of birth, place of birth, and passport number if applicable. Also, include the country of issuance, date of issuance, and expiration date for the passport.
4. In Section III, indicate whether your child will be traveling alone or with an individual/organization. If traveling with someone, provide their name, relationship to the child, and any relevant passport details.
5. For Section IV, specify the travel destination and dates during which your child is authorized to travel.
6. Finally, complete Section V by signing and dating where indicated for each parent or legal guardian.
