2. Begin with SECTION I by entering the Escrow Company Information. Fill in the Company Name, Address (Street Address or P.O. Box, City, State, Zip), and the Escrow Agent's Name.
3. Move to LIENHOLDER INFORMATION in SECTION II. Select the Lienholder Status by checking one of the options: Legal Owner, First Junior Lienholder, or Second Junior Lienholder. Then provide the Lienholder Name and Address details.
4. In SECTION III, input the REGISTERED OWNER(S) INFORMATION including Name(s) and Loan Number.
5. Proceed to SECTION IV for DESCRIPTION OF UNIT. Enter Manufacturer Trade Name, Manufacture Year, Manufacturer Serial Number(s), and Decal (License) Number(s).
6. Complete SECTION V by certifying under penalty of perjury. Fill in the total payment amount required for lien release and expiration date.
7. Finally, in SECTION VI, have the Escrow Agent certify that payment has been made in full according to terms outlined.
