3. Proceed to SECTION B: NEW AGENT DETAILS. Enter the Agency Name, ACIC number, Office Address, Email, Contact Person's name, and Phone number. Use the checklist to indicate which stages of the application process the new agent assisted with by checking the relevant boxes.
4. If changing agents, complete SECTION C: CHANGE OF AGENT. Provide details of the Previous Agency Name, Contact Person, Previous Agency Office Address, and Reason for Change of Agent.
5. In SECTION D: AGENT DECLARATION, sign and date to confirm that all information is accurate and authorized by the student.
6. Finally, in SECTION D: STUDENT DECLARATION, have the student print their name and sign along with a witness signature and date.
