In Section A, provide your details as the applicant. Fill in the total investment amount, number of jobs created, and existing or estimated tax revenue generated by your project.
Sign and date Section A to certify that all information is accurate. Ensure you include your title and phone number.
Next, forward the form to the local legislative authority for completion of Section B. This section requires endorsement from a city, village, or township official.
Once Section B is completed and signed, return the entire form to yourself. Attach it to your transfer application (DLC4120 or DLC4118) for submission.
