What Does this Exam Consist of? A DOT Physical evaluates a drivers vision, blood pressure, and a complete review of their medical history. This can help ensure that a driver does not have certain health conditions and can safely operate a commercial vehicle.
The DOT physical requires drivers to have 20/40 vision with or without the use of corrective lenses. This means you must be able to see at least 20/40 with both eyes, or at least 20/40 with either the right eye or left eye. Those with monovision may be disqualified from getting a DOT medical card.
The DOT drug test will screen for cocaine, amphetamines, opioids, THC, marijuana, and PCP. If the driver tests positive for any of these drugs, additional follow up may be required to determine if there is an alternative medical explanation for the substances which were identified.
About the Medical Examination Report (MER) Form, MCSA-5875: The Federal Motor Carrier Safety Administration requires that interstate commercial motor vehicle drivers maintain a current Medical Examiners Certificate (MEC), Form MCSA-5876 to drive.

If the Medical Examiner determines that the driver examined is physically qualified to drive a commercial motor vehicle in ance with the physical qualification standards, the Medical Examiner will complete and provide the driver with a Medical Examiners Certificate (MEC), Form MCSA-5876.
Drivers engaged in Excepted transportation, including but not limited to school bus drivers, government employees, beekeepers (apiarists), etc., do not have to submit the Medical Examiners Certificate (DOT Physical Card) to PennDOT. This is not a new requirement.
