Most 1996 and newer gasoline-powered passenger vehicles are subject to emissions inspections after they are four years old (e.g. 2012 vehicles are being inspected in 2016 for the first time). The inspection month coincides with the expiration date of the vehicle license plate.

276.401. Waiver Requirements After failing a retest, the vehicle has failed to comply with the applicable vehicle emission standards; A minimum expenditure of at least $450 in emission-related repairs exclusive of tampering-related repairs have been made;
You cannot renew your cars registration if you do not take an emissions test. The rule does not apply to diesel trucks, and completely electric vehicles. Motorcycles and antique vehicles (model year before 1967) are also exempt. If your vehicle is registered outside of Illinois, you also are exempt.
