5. Fill out Section E regarding your parents' marital status at the time of your birth.
6. If applicable, complete Section F with details about the celebrant who will solemnise your marriage.
7. In Section H, ensure you attach all necessary documents such as birth certificates and any divorce decrees if previously married.
8. Finally, review all entries for accuracy before signing the declaration at the end of the form.
