The Activity Measure for Post-Acute Care (AM-PAC\u2122) "6-Clicks" Inpatient Short Forms are multidimensional measures that use 6 questions to assess functional outcomes of patients in postacute care settings.
Scores range from 6 to 24, with higher scores representing greater functional independence. We characterized the association between 6-clicks score and PAC disposition (home, home care, or SNF) in patients admitted for HF whose relative functional status was moderate (score >12) or better.
One \u201c6-Clicks\u201d form assesses basic mobility function, and another form assesses daily activity function. Each item is scored from 1 to 4 on the basis of the amount of difficulty a patient has or how much help is needed from another person in completing the task. Lower scores equate to lower levels of function.
The original AM-PAC\u2122 is used with a variety of diagnoses and patient populations to measure basic mobility (with and without a stair-climbing option), daily activity, and applied cognitive functions. The AM-PAC\u2122 measures aspects such as difficulty, assistance, and limitations in activities of daily living.
