How to use or fill out the ejection form with our platform

- Begin by filling in the sport (M/W) and site information at the top of the form. This helps identify the context of the ejection.
- Enter the date of the incident, along with details about both home and visiting teams.
- Indicate whether it was a violent or non-violent ejection by checking the appropriate box. Also, provide totals for each type of ejection for the individual during the current season.
- Fill in the name of the ejected person and specify their role (Head Coach, Asst. Coach, Player, etc.).
- In the 'Reason(s) for Ejection' section, describe in detail why the individual was disqualified from the contest. Use additional pages if necessary.
- Finally, ensure that you sign and date the form as an Athletic Director before submitting it to your region director.
