5. Record the Date of Observation and Time/Period to document when the observation took place.
6. For each component listed, rate your observations by selecting one of the options: Ineffective, Developing, Effective, Highly Effective, or Not Applicable.
7. Proceed to the ASSESSMENT OF PREPARATION AND PROFESSIONALISM section and repeat similar steps for components 1a, 1e, and 4e based on prior observations.
8. Add any additional evaluator notes as necessary and ensure both teacher and evaluator sign and date at the bottom of the form.
