Click ‘Get Form’ to open the remserv online claim form in the editor.
Begin by filling in your personal details in the 'My details' section. Include your full name, RemServ ID, payroll number, date of birth, email, contact number, and address.
Proceed to the 'My claim details' section. Here, indicate what you would like to claim and specify whether the payment should go to you or a supplier. Fill in the required account details including BSB and account number.
For each expense, select how soon you want the payment processed (ASAP, weekly, fortnightly, or monthly) and provide any necessary references or biller codes.
Review your entries against the checklist provided at the end of the form to ensure all required documents are included before submission.
