The Elderly and Persons with Physical Disabilities (EPD) Waiver Program allows DC residents, who would otherwise require nursing home care, to receive services and supports while living in their home or in assisted living communities.
As discussed earlier, Skilled Nursing Facilities provide more complex medical care and rehabilitation while Long Term Care Facilities offer more permanent support for day-to-day needs. In some instances, both types of institutions are combined to provide the most comprehensive level of care.
Who can Participate? DC residents with IDD who are at least 18 years of age are eligible to participate in My Life, My Way. Participants must: live in their natural home, be eligible to receive Medicaid, and be enrolled in the Individual and Family Support (IFS) wavier.
DC Medicaid helps low-income city residents pay for medical care. Sometimes it helps with in-home care. And sometimes it pays for assisted living. Medicaid also pays for long- term care in nursing homes.

The DC HealthCare Alliance program offers medical assistance to DC residents who do not qualify for Medicaid or Medicare, or do not have any other health insurance. If you have low income, you may qualify to become a member of DC HealthCare Alliance. Your income must be at or below 200% of the federal poverty level.
If you would like assistance with the application process, please call us at 202-724- 5626. Your Medicaid provider will complete this form to initiate a face-to- face assessment by Liberty Healthcare. This is your application to the EPD Waiver program.
The EPD Waiver is for District of Columbia residents who are elderly (65+ years old), or physically disabled and between 18-64 years of age. Persons who enroll as a disabled adult can continue to receive waiver services under the aged category when turning 65 years old.
