California removed its personal and religious exemption option in 2015. Parent/guradian must complete an online educational module to receive a non-medical exemption. Connecticut removed its religious exemption option in 2021.
The parent or guardian simply provides their childs name, date of birth and states their decision to exempt their child from the school vaccination requirements, and files this with the day care operator or school principal. Medical exemptions are completed by the childs healthcare provider.
Students must meet immunization requirements as stated in Louisiana law to legally attend school.
