1. Click ‘Get Form’ to open the application for CISA Certification in the editor.
2. On Application Page A-1, fill in your personal details including name, address, and contact information. Ensure you read the acknowledgment section, then print and sign your name along with the date at the bottom.
3. Proceed to Application Page A-2. In Section A, list your work experience in Information Systems Audit, Control, Assurance or Security. Include employer names, dates of employment, and duration of experience.
4. In Section B, indicate any experience substitutions you are claiming. Provide details about non-IS audit or information systems work experience if applicable.
5. For Section C, check the box for any educational waivers you are applying for and attach necessary documentation such as transcripts or degrees.
6. Complete Sections D by summarizing your total work experience and ensuring it meets the minimum requirement of five years.
7. Fill out Verification of Work Experience forms on Pages V-1 & V-2. Ensure that verifiers complete their sections accurately before submission.
8. Collect all supporting documents and submit your completed application via fax, email or mail to ISACA's Certification Coordinator.
