- Indicate maintenance frequency and list utilities provided by marking 'O' for Owner or 'T' for Tenant as applicable.
- Complete any additional sections regarding upgrades and improvements since last contract rent determination.
- Finally, sign and date the form before submitting it via email or fax as indicated at the bottom of the document.
