Before proceeding, please answer the questions on the Criteria/Information Sheet 2026

4. Provide participant or tenant information including Client #, Tenant’s Name, Address, Unit #, Zip Code, and Phone Number. Remember to attach a 60-Day Notice.
5. In the Property & Unit Description section, select the structure type and complete details about unit size including bedrooms, bathrooms, square footage, year built, and amenities.
6. Indicate maintenance frequency and list utilities provided by marking 'O' for Owner or 'T' for Tenant as applicable.
7. Complete any additional sections regarding upgrades and improvements since last contract rent determination.
8. Finally, sign and date the form before submitting it via email or fax as indicated at the bottom of the document.
