2. Begin by filling out the Parent/Admin Information section. Enter your first name, last name, relation to the player, and contact details including home, work, and cell phone numbers. Ensure that you provide a valid email address.
3. Next, move to the Player Information section. Indicate whether the player is new or returning by selecting the appropriate checkbox. If returning, include their Cal South Player ID number.
4. Fill in the player's first name, last name, gender, date of birth (MM/DD/YYYY), height, weight, school name, grade, league, club, shirt size, sock size, and age group division.
5. Complete the Emergency Contact sections by providing names and phone numbers for two emergency contacts.
6. Lastly, review any medical problems or physical limitations and indicate your parental/volunteer support preferences before signing and dating the form.
