How do I make an ACC claim? You claim by lodging an ACC 45 Injury Claim Form. The form must always be completed by a treatment provider such as a doctor, physiotherapist or ambulance driver. Usually they will send the form to ACC for you.
A medical practitioner or nurse practitioner can issue a medical certificate as part of the initial ACC45 injury claim, which covers your patient for up to the first 14 days. The ACC18 can be used to certify beyond the first 14 days.
To lodge a sensitive claim: use the Read code SN571 (Sexual Abuse), or use 'Z' and add any other relevant injury codes as needed. write 'sensitive claim' in the accident description field.
To get a medical certificate. You'll need to first book an online appointment with a Tend doctor. We'll ask you some questions about your current state of health and get an understanding of what's required on your medical certificate. In some cases, we'll need you to visit the clinic to issue your doctor's certificate.
If your life-long injury gets worse or you have a new injury, you can apply again using the same process. You can do this once every twelve months.

The ACC18 form is also used to indicate medical clearance, in other words that your patient is now fully fit to work. This may happen when they're still undergoing treatment for their injury.
For standard claims, ACC has 21 days after receiving a claim to process that claim and make a decision about your entitlement. They can extend the time for making the decision if they need more information, but they can't extend it to more than four months after you lodged your claim.
You have to lodge your claim for ACC cover within 12 months after the date of your accident or injury.
Your injury must be because of an accident. We won't cover things like illness, conditions from ageing and emotional issues.
