This form, approved by the Judicial Conference of the United States in September 1974, is required for the use of the Clerk of Court for the purpose of initiating the civil docket sheet. Consequently, a civil cover sheet is submitted to the Clerk of Court for each civil complaint filed.
In the Judiciary Act of 1789 Congress established a system of circuit courts to serve as the trial courts for most federal criminal cases, for suits between citizens of different states (diversity cases), and for civil suits initiated by the United States.
A cover page should look like a business letter. It should be professional, clearly formatted, and no longer than one page, with a focus on why youre a strong fit for the role. It should have the same contact information (name, phone number, email address) as your resume.
