Begin by selecting one of the appeal options. Indicate whether you are appealing a decision from a USCIS officer or another type of DHS decision. Fill in the required fields such as the name of the beneficiary and their A-number if applicable.
In the next section, provide details about the DHS officer's decision you are appealing, including their title and the date of the decision. This information is crucial for processing your appeal.
Clearly specify your reasons for appealing. If necessary, continue on separate sheets. Ensure that your reasoning is detailed to avoid summary dismissal of your appeal.
Indicate whether you desire an oral argument before the Board and if you plan to file a separate written brief after submitting this notice.
Finally, sign and date the form, ensuring all personal information is accurate before submission.
