6. List any new medicines prescribed under 'Name(s) of new medicine(s)'. Ensure you check all applicable conditions like Asthma/COPD or Diabetes.
7. Document progress records for initial interventions and follow-ups, indicating methods used such as face-to-face or telephone.
8. Complete sections on healthy living advice provided and reasons for non-participation by ticking appropriate boxes.
