3. Select your payment frequency by checking either 'Weekly' or 'Bi-Weekly'.
4. Read through the Authorization Agreement carefully. This section authorizes FirstEnergy Corp. to manage your direct deposit entries.
5. Complete the Primary Account Information section. Choose whether this is a New authorization, Change in Existing, or Cancel. Enter your Financial Institution Name, select Checking or Savings, and provide the Routing Number and Account Number.
6. If applicable, fill out the Expense Reimbursement Account Information using the same process as above.
7. For any Other Account Information, repeat the steps for entering details as needed.
8. Sign and date the form at the bottom to validate your authorization.
9. Attach a voided check or photocopy of a check (not a deposit slip) for verification purposes.
10. Finally, send your completed form to Payroll Services at J-JOHN or fax it to 814-534-4083.
