4. In Section C, provide vehicle details including license number, identification number, make, and select the appropriate weight code based on the highest weight range your vehicle will operate under.
5. Indicate if the vehicle will operate under 10,001 lbs by placing an (X) in the designated box. Also, enter the date when each vehicle will first operate at this declared weight.
6. Finally, complete Section D by certifying your declaration with your signature and date at the bottom of the form.
