A V62 form is an official DVLA document that is used to make an application for a replacement V5C registration document if you have not been given one when purchasing a new vehicle, or if a V5C has not arrived from DVLA within 6 weeks of registering the vehicle.
The V5C logbook (also known as the V5 form or document) records the Registered Keeper (or Keepers) of the vehicle. Legally, that's the person registering and taxing the vehicle, not necessarily the owner. Normally the owner and registered keeper will be the same, but there are occasions where they might differ.
Use form V62 to apply for a vehicle registration certificate (V5C). You can get a duplicate vehicle log book (V5C) online if you do not need to change anything in the log book. If you need to make any changes or you're not yet registered as the vehicle's keeper, you must make an application by post using form V62.
