Section 8 requires you to classify your products; mark an X according to whether they are Commercial Production Stock, Multiplier Breeding Stock, or Primary Breeding Stock.
For classification in section 9, enter an X for each NPIP classification that applies. Note that 'S. Enteritidis Monitored' should not be checked.
Complete sections requiring signatures from State Inspectors (lines 11 and 13) only after verification of classifications has been completed.
