5. Enter the MEIS account of the authorized official in the designated field.

7. The superintendent must attest to the authorization by filling in their name, signing, and dating the section at the bottom of the form.
8. Once completed, forward the form to the Office of Career and Technical Preparation using the provided fax number or mailing address.
