ECFMG Certification is the standard for evaluating the qualifications of international medical graduates (IMGs) entering the U.S. health care system. The Accreditation Council for Graduate Medical Education (ACGME) requires IMGs who enter ACGME-accredited residency or fellowship programs to be certified by ECFMG.
Also, the foreign medical degree or license must be equivalent to an M.D. in the U.S. An ECFMG certificate may be sufficient as evidence that the foreign medical degree or license is equivalent to an M.D. but the immigration service may still request an educational equivalency degree evaluation.
Permanent Validation of ECFMG Certificate successfully complete at least 12 months of clinical education in a U.S. non-standard training program associated with an ACGME-accredited program; or. hold an unrestricted license to practice medicine in a U.S. state or the District of Columbia.
To request an extension, you must complete a Request to Extend a USMLE Step 1/Step 2 CK Eligibility Period as described below in the Overview. Payment of the appropriate fee must accompany the request. You may request an extension of your eligibility period only once for each exam registration.
After ECFMG certification, physicians who wish to practice medicine in the United States must complete an accredited residency training program in the United States or Canada. This process will take at least 3 years.

ECFMG requires that international medical graduates satisfy the examination requirements for ECFMG Certification within a seven-year period. This means that once you pass an exam, you will have seven years to meet the other examination requirements for ECFMG Certification.
We will make your Certificate valid indefinitely in our records on the basis of your J-1 visa status after your successful completion of at least 12 months of eligible clinical training and notify you when the process is complete.
