Patient Eligibility: Any Ontarian with a valid Ontario Health Card, living in Ontario, and diagnosed with type 1 or type 2 diabetes. Not required to be taking 3 chronic prescription medications but patient must be diagnosed with either type 1 or type 2 diabetes and taking medication for their condition.
MedsCheck and Diabetes MedsCheck services must be claimed online via the Pharmacy Programs Administrator Portal by the end of the calendar month following the services (e.g. services undertaken in May must be claimed by 30 June). Claims cannot be submitted outside this timeframe and will not be paid.
What is a medication review? A medication therapy review is a one-on-one consultation between a pharmacist and a patient during which the pharmacist collects information about the patients health conditions and all of the medications he/she takes whether prescribed, over-the-counter, or a natural health product.
One MedsCheck/Diabetes MedsCheck service can be conducted per eligible Patient per 12 months.
