2. Begin by entering the name and address of the hospital in Part 1. Ensure that you print the full name of the patient clearly.
3. Determine if the patient is already an in-patient. If not, complete section (a) by filling in the date and time of admission, along with the relevant section of the Mental Health Act 1983.
4. If the patient is already admitted, complete section (b) with the date and time when the application for admission was received, along with the applicable section of the Act.
5. Sign on behalf of the hospital managers and print your name along with today's date.
6. If applicable, proceed to Part 2 to document any emergency admissions under section 4. Fill in the date and time when you received the second medical recommendation.
