- Click ‘Get Form’ to open the Payment Authorization Form in the editor.
- Begin by entering your primary applicant name and policy number(s) at the top of the form.
- Select your preferred payment type: either 'Automatic Credit Card Payment' or 'Automatic Bank Draft/ACH Payment'. If you choose credit card, proceed to Section A; if bank draft, go to Section B.
- For credit card payments, fill in the card type, name as it appears on the card, card number, and expiration date in Section A.
- For bank draft payments, complete Section B by selecting account type (checking or savings), entering account holder name, bank name, routing number, and account number.
- In Section C, sign and date the form to authorize the payment method selected. Ensure you understand the terms regarding cancellations and NSF charges.
- Once completed, return the form via email to policyservices@ihcgroup.com or fax it to 602-906-4745.
