Depending on where you work, your facility may still utilize the FIM but many facilities have begun to use the CARE, Continuity Assessment Record and Evaluation, Item Set. This tool was adopted by Medicare to be used as a standardized assessment tool at acute discharge and post acute admission and discharge.
The FIM Scale The scale comprises 18 items, which are divided into two main categories: motor and cognitive functions. Each item is scored on a scale of 1 to 7, where 1 indicates total dependence and 7 indicates complete independence.

The FIM measures the level of assistance required to perform 18 ADL tasks (see Table I) [19]. The tasks are categorized as either motor (13 tasks) or cognitive (5 tasks). Each task is scored on a 7-point ordinal scale to measure independence as determined by the amount of assistance required to perform each ADL task.
The Functional Independence Measure (FIM) is an outcome measurement tool that doctors, nurses, and therapists use to assess patients ability to complete specific functional tasks while recovering from a stroke, traumatic spinal cord injury, traumatic brain injury, or cancer.
