In Box 1, assess acute onset and fluctuating course. Answer questions a) and b) by selecting 'Yes' or 'No' based on your observations of the patient's mental status changes throughout the day.
Next, evaluate Inattention by answering whether the patient had difficulty focusing attention. Mark your response accordingly.
Proceed to Disorganized Thinking. Indicate if the patient’s thoughts were disorganized or incoherent by checking 'Yes' or 'No'.
In Box 2, rate the patient's level of consciousness using the provided options from Alert to Coma. Make sure to select one that best describes their state.
Finally, review your selections. If Inattention and at least one other item in Box 1 are checked along with one item in Box 2, a diagnosis of delirium may be suggested.
