The NPRC will provide an equivalent DD Form 214 Member-4 or an NA Form 13038, which is equivalent to the DD Form 214 Member-4. (The NA Form 13038 does not provide Home of Record or Mailing Address After Separation.)
Veterans or their next of kin who need assistance in obtaining service records, a DD 214, or replacement awards and medals must formally request them through the National Personnel Records Center (NPRC). The NPRC is the records custodian for most discharged and retired members of all branches of service.
The National Archives opens all records to the public 62 years after discharge. If the Veteran was discharged less than 62 years ago, you may be able to request limited information from their Military Personnel File. Youll need authorization from the Veterans next of kin to request their full military service record.
A non-archival OMPF is open to the Veteran, the next of kin or to a third-party requester who has the Veterans written authorization. Under the provisions of FOIA, the general public may only obtain limited information from these records. Requests for non-archival records must be signed and dated.
Obtain the NARA NA 13055 form from the National Archives website or a local branch. Fill in your personal information at the top of the form, including your full name, address, and contact details. Indicate the purpose of your request in the designated section.
