- Next, provide information about the placement. Fill in the name of the placement organization, employment sector, supervisor's name and contact details.
- In Section B, specify the period of agreement by entering start and end dates. Clearly outline placement hours and identify working days.
- For Section C, indicate who will provide Workplace Safety & Insurance Board coverage during the training period. Select either 'the placement' or 'the Ministry of Education' as applicable.
- Finally, ensure all parties sign in Section D to validate the agreement. This includes signatures from the student, parent/guardian, placement representative, and teacher.
