Click ‘Get Form’ to open the ckc online application in the editor.
In the CANINE SECTION, enter the NAME OF APPLICANT DOG using one box per letter. Fill in the CALL NAME, BREED, COLOR, and BIRTHDATE (MM/DD/YY). If applicable, include MICROCHIP ID # and any optional details like BLOODLINE and MARKINGS.
Select the SEX of your dog by checking either MALE or FEMALE. If you have information about the breeder or DNA certification, fill those fields as well.
In the OWNER SECTION, select your title (MR., MS., MRS.) and provide your FIRST NAME, LAST NAME, and EMAIL ADDRESS. Ensure you check if you are an existing CKC member.
Complete your MAILING ADDRESS including APT/ST#, CITY, STATE, ZIP CODE, and PHONE NUMBER. Don’t forget to sign and date the application at the bottom.
In the PARENT SECTION, provide details for both sire and dam if applicable. Attach copies of their registration certificates if they are not registered with CKC.
Finally, navigate to the PAYMENT SECTION to enter your payment details before submitting your application via mail, fax, or email.
