The Department of Motor Vehicles (DMV) in New York can send you a paper copy of a police report upon request. These mailed reports cost $25. You may need help filling out some of the required forms to make a request, so having a lawyer by your side is beneficial.
Schedule an appointment. Once you have an appointment, visit the Certificate of Conduct and Non-Criminal Fingerprint section (Room 152-A) at One Police Plaza in lower Manhattan. The office is open Monday through Friday, 8 AM to 4 PM. Processing hours are 8 AM to 3 PM.
Requests for copies of an MV-104A for collisions investigated by the New York State Police that occurred anywhere in New York State on roads or highways other than the Thruway must be made to DMV. The form required (MV-198C) to request a copy of the report is available online at: .dmv.ny.gov.
