2. Begin by entering your 13-digit Bar-Coded Identity Document or Passport Number, followed by your Date of Birth, Gender, First Names, and Surname.
3. Fill in your Postal Address and Residential Address along with the corresponding codes. Ensure you provide a valid Telephone Number and Cell Number.
4. Indicate your Occupation and Occupation Code. Provide your E-Mail Address and Fax Number if applicable.
5. Select your preferred Method of Payment: Cheque, Paypoint, or Bank Transfer. If using Bank Transfer, refer to the UI-2.8 form for Banking Details.
6. Complete details regarding any previous applications and indicate if you are still employed. If yes, ensure to complete Form UI-2.7 as well.
7. Enter the Date of Commencement of Maternity Leave and if applicable, the Date you returned to work.
8. Provide information about other sources of income by marking applicable options and detailing when you began receiving this income.
9. Have a qualified medical practitioner or registered midwife complete the Medical Certificate section confirming your pregnancy or recent childbirth.
10. Finally, sign and date the application before submitting it for processing.
