Begin by entering the 'Name of Applicant' in the designated field. Ensure that this is the official name of your batching plant.
Fill in the 'Address' and 'Telephone Number(s)' for your organization. This information is crucial for communication purposes.
If your batching plant's address differs from the applicant's, provide that information in the respective fields along with its telephone number.
Enter the 'Name' and 'Position' of your duly authorized representative. This person will be responsible for liaising with DPWH.
List all machinery and laboratory equipment under 'Plant Machinery and Laboratory Equipment/Apparatus'. Include details such as purpose, manufacturer, acquisition date, and service date.
Outline your calibration program for measuring instruments in the specified section. Use a separate sheet if necessary.
Provide a list of personnel along with their qualifications, categorized into production and quality control personnel.
Finally, ensure to sign where indicated as President/Manager/Duly Authorized Representative before submission.
