As part of Indiana University's pandemic response, all staff, faculty and students whose work or class schedule requires them to be regularly present on campus will be required to get an annual flu vaccine during the fall 2020 semester.
All current students, faculty and staff will use IU's COVID-19 vaccine report form to attest that they are vaccinated and report all doses of your vaccine. Incoming students should plan to complete the self-report form once have created their IU email account.
Students may be required to complete a physical examination (see program-specific requirements. Additional immunizations may be required at certain clinical sites. Students assigned to those sites must complete additional requirements prior to starting that clinical rotation.
Go to the Med+Proctor website or search for \u201cImmunization Compliance\u201d in One. IU to create an account and upload proof of compliance. Once you create a Med+Proctor account, you will have two options for submitting your documentation: Pay a one-time fee of $10 for lifetime access to any immunization records you upload.
