6. Fill in the GENERAL SERVICE REPRESENTATIVE (G.S.R.) section with your name, address, and telephone number. If applicable, also complete the ALTERNATE G.S.R. section.
7. Decide whether to list your group in the Directory by checking 'Yes' or 'No'.
8. Finally, print and sign the document or type your name before saving it as a PDF and emailing it to registrarOA58@aa-oregon.org.
