Georgia law requires children attending a school or a childcare facility be protected from certain vaccine-preventable diseases. Parents will need to provide Georgia Immunization Certificate (Form 3231) to their childs school or child care to show their child has been vaccinated against certain preventable diseases.
No child or person shall be admitted to or retained in any public, private, or parochial school, grades kindergarten through twelve (K-12), or any public or private childcare facility as defined in Code Section 63-13-20 without a valid South Carolina Certificate of Immunization.
All students entering school are required by State law to have a South Carolina State Certificate of Immunization (DHEC 4024). South Carolina Department of Health and Environmental Control (SCDHEC) Immunization requirements for the 2023-2024 school year may be found here.
