Behavior and Symptom Identification Scale (BASIS-24) BASIS-24 was developed at McLean as an outcome assessment tool for adult mental health patients. It is used across all McLean inpatient, residential, and partial programs. This patient self-report survey consists of 24 standardized items.
The consumer responds by selecting how often they are experiencing difficulty with a corresponding score of: 0 - No difficulty 1 - A little difficulty 2 - Moderate difficulty 3 - Quite a bit of difficulty 4 - Extreme difficulty Scoring of this measure is conducted through a proprietary, licenced algorithm.
The 24 questions are scored on a 5-point scale (from 0 to 4) and each subscale and overall mean scores also range from 0 to 4, with 0 being the lowest severity of symp- toms and 4 being the highest severity of symptoms.
