Department of Defense Active Duty/Reserve/Guard/Civilian Forces Dental Examination Form (DD Form 2813) This form is used to used to assist active duty and National Guard and Reserve members in documenting dental health for worldwide duty. It should be downloaded and given to the dental provider to complete.
If you have cavities, you must get them filled before being sworn into the military. If you need a root canal, you can still join the military through the delayed entry program. Bad breath is not considered grounds for insufficiently healthy teeth so this will not impact your ability to join the military.
If your dental implants fix an issue that is no longer recurrent, you can get an approved waiver to join the military. However, if you have multiple dental implants or your implants impact your ability to chew or eat a normal diet, then this is disqualifying.
This form is used to determine dental readiness as it relates to a National Guard and Reserve members worldwide deployment status. It answers the question: Is this NGR member dentally ready to deploy for worldwide duty?
IRs must have a current dental exam and be either dental class 1 or 2 to be classified as medically ready. An in-person dental exam at a military facility is required every three years.
