2. Begin with Participant #1's section. Fill in the full name as it appears on the passport, including first, middle, and last names.
3. Enter the passport number, date of birth (mm/dd/yy), country of issue, and expiration date of the passport.
4. Complete the mailing address fields: street address, city, state, zip/postal code, and country. Include day phone, evening phone, cell phone, and email.
5. Provide emergency contact information by filling in the relationship, name, and their contact numbers.
6. For medical and dietary information, specify height, weight, shoe size, gender, allergies, dietary restrictions/preferences, current medications with reasons, physical limitations affecting participation, health condition description, exercise routine details, smoking status during the trip and any additional medical information.
7. Repeat steps 2-6 for additional participants if necessary.
