1. Click ‘Get Form’ to open the cancer request form in the editor.
2. Select the appropriate location for the referral: Palo Alto, South Bay, or Redwood City. Fill in the date and number of pages faxed.
3. Provide referring provider information, including the name of the referring MD, medical group, phone, fax, and address details.
4. Complete patient information by entering the patient's last name, first name, middle initial, date of birth, gender, phone number, height, weight, and address.
5. Indicate if an interpreter is needed and specify the language. Note any special assistance required.
6. In the reason for referral section, choose a specialty and provide diagnosis/ICD10 code along with service/specialty requested.
7. Fill in details about the physician requested and current insurer. Indicate if authorization is required.
8. Select type of service requested (e.g., clinic consultation or surgery) and add any relevant documentation to support diagnosis.
9. Use the additional comments section for any further notes or specific requests related to patient care.
