- Provide feedback in the 'Strengths' and 'Suggestions for development' sections. If necessary, rate clinical competencies using a scale from Unsatisfactory to Excellent.
- Finally, ensure both Assessor and Registrar sign and date at the bottom of the form before submitting it.
